The Perry Police Department offers record request services from 8:00 am to 5:00 pm, Monday through Friday (excluding holidays) located at 1207 Washington Street in Perry.

Appropriate release forms and proof of identity are required. Contact Kayla Neesmith at 478-988-2807 or kayla.neesmith@perry-ga.gov.

Incident & Accident Reports

Copies of police Incident or Accident Reports are available from 8:00 am to 5:00 pm, Monday through Friday (excluding holidays) located at 1207 Washington Street, Perry.

A processing fee is charged for each report.
